2008 Season Registration Information and Forms

Thank you to those players and parents who attended the informational meeting on Monday, November 5th.  It was great to see all of you and hope that we were able to answer your questions about the club..

The registration fee on or after February 1, 2008 is $475 per player.  Families with two or more players will receive a $100 discount on each player after the first player has paid the full fee.  Discounts earned for Ribfest Parking (July 2007) and ButterBraids sales (November 2007) will help reduce a player's registration fee. 

All players must complete and submit the five (5) forms below before being allowed to practice with the club:

  1. Player registration form (all 3 pages)
  2. Parent Volunteer form
  3. NCHS Redhawks Lacrosse Release/Waiver Form
  4. Naperville Central Athletic Form (Your son can obtain this form from the NCHS Athletics office.  This is the preferred method as this form will be on heavier card stock preferred by the school  As a last resort, you may use this PDF file.)
  5. ISHA Physical form
    • Players who played a spring, fall, or winter sport during 2007 can pick up a copy of their IHSA physical from (if needed) from the NCHS Athletics office (see Debbie Adams).
    • All players MUST be covered by a physical exam that is less than one year old throughout the season.  If the player's last physcial was:
      • On or after June 1, 2007 - he is covered for the season and only needs to submit the IHSA physcial form to the club's registrar.
      • Between February 25, 2007 and May 31, 2007 - he is able to start practices and games, but must get another physical and submit the IHSA physical form to the club's registrar before the previous physical is a year old.
      • Before February 25, 2007 - player must have another physical exam and submit the IHSA physical form to the club's registrar before he is able to participate in practices.

NO EXCEPTIONS to the above procedure regarding physical exams

New players to the club also need to submit a Uniform Order form:

  • Uniform Order form - All NEW players must purchase a uniform
    • Uniform jerseys will be $40.  XXL and larger will be $45.
    • Uniform shorts will be $35.  XXL and larger will be $40.
    • Replacement reversible practice pennies will be $21.
    • Returning players who want to replace a practice penny, uniform jersey, and/or uniform shorts, or are requesting a different uniform number, need to submit a Uniform Order form with proper payment.
    • Players will be assigned their jersey numbers starting December 3, 2007 after they have paid for their uniform.
  • Available Uniform Numbers to choose from (as of January 16, 2008).  These uniforms have already been ordered and are available for purchase for players who decide to join the club after January 27th:

No uniform numbers are currently available.  We are working on getting additional uniforms for new team members.


Due date for Registration Fee and forms was Wednesday, January 31, 2008.  All forms and fees ARE DUE NOW.  Forms and fees can be dropped off or mailed to the registrar:
