2010 Season Registration Information and Forms
The team has schedule an informational meeting for September 1, 2009 to review and answer questions about Registration materials.
Registration forms ARE NOW on the website (see below).
Registration fee for 2010 for returning players is $550.00. The fee for new players to the team is $575.00 and includes a one-time uniform maintenance fee. Players will also have to submit a $100.00 uniiform deposit fee to ensure uniforms are returned at the end of the season.
Families with two or more players will receive a $100 discount on each player after the first player has paid the full fee. Discounts earned for Ribfest Parking (July 2009) and any other fundraising will help reduce a player's registration fee.
All players must complete and submit ALL of the forms below before being allowed to tryout and practice with the team:
- Player registration (all 3 pages), Waiver, Code of Conduct, Parent Duties, and Uniform forms
- Uniform and replacement Penny Order form - All players must confirm their previous uniform number (and size) and give their SHORTS size
- Replacement reversible practice pennies will be $21.
- Returning players who want to replace a practice penny need to submit a Uniform Order form with proper payment.
- New players will be assigned their jersey numbers starting February, 2010.
- Naperville Central Athletic Form (Your son can obtain this form from the NCHS Athletics office. This is the preferred method as this form will be on heavier card stock preferred by the school. You can also use this PDF file.)
- ISHA Physical form
- Players who played a spring, fall, or winter sport during 2009 can pick up a copy of their IHSA physical from (if needed) from the NCHS Athletics office (see Debbie Adams).
- All players MUST be covered by a physical exam that is less than one year old throughout the season. If the player's last physcial was:
- On or after June 1, 2009 - he is covered for the season and only needs to submit the IHSA physcial form to the team's registrar.
- Between March 1, 2009 and May 31, 2009 - he is able to start practices and games, but must get another physical and submit the IHSA physical form to the team's registrar before the previous physical is a year old.
- Before March 1, 2009 - player must have another physical exam and submit the IHSA physical form to the team's registrar before he is able to participate in practices.
NO EXCEPTIONS to the above procedure regarding physical exams
Due date for registration fee and forms will be February 5, 2010. All forms and fees will be due at that time. Make checks payable to "Naperville Central Boys Lacrosse". Forms and fees can be dropped off or mailed to the registrar:

P.S. Refunds
It is the team's policy to provide a refund at the following percentages and times:
Prior to first practice - Full refund, less any expenses already incurred by the team for the player. These expenses could include practice penny, insurance, etc.
Player is cut from team during tryouts - Full refund, less any expenses already incurred by the team for the player. The team will attempt to limit these expenses, especially if player cuts are expected due to the number of players trying out.
Prior to first game - 50% refund.
After first game - No refund (unless injured). No refund will be granted after the first game if the player
- Quits the team
- Is suspended by the team
- Or becomes ineligible to play due to grades (academic ineligibility).
