2009 Season Registration Information and Forms

The club will be scheduling informational meetings for early November and early December.  Registration packets will be passed out at the November meeting and registration will be due in early to mid-January. 

Registration fee for 2009 should be in the $450 range.  Families with two or more players will receive a $100 discount on each player after the first player has paid the full fee.  Discounts earned for Ribfest Parking (July 2008) and ant other fundraising will help reduce a player's registration fee. 

All players must complete and submit the five (5) forms below before being allowed to practice with the club:

0. Player registration form (all 3 pages) (currently 2008 form )
0. Parent Volunteer form  (currently 2008 form )
0. NCHS Redhawks Lacrosse Release/Waiver Form (currently 2008 form )
0. Naperville Central Athletic Form (Your son can obtain this form from the NCHS Athletics office.  This is the preferred method as this form will be on heavier card stock preferred by the school  As a last resort, you may use this PDF file.)
0. ISHA Physical form
   • Players who played a spring, fall, or winter sport during 2008 can pick up a copy of their IHSA physical from (if needed) from the NCHS Athletics office (see Debbie Adams).
   • All players MUST be covered by a physical exam that is less than one year old throughout the season.  If the player's last physcial was:
     • On or after June 1, 2008 - he is covered for the season and only needs to submit the IHSA physcial form to the club's registrar.
     • Between February 25, 2008 and May 31, 2008 - he is able to start practices and games, but must get another physical and submit the IHSA physical form to the club's registrar before the previous physical is a year old.
     • Before February 25, 2008 - player must have another physical exam and submit the IHSA physical form to the club's registrar before he is able to participate in practices.

NO EXCEPTIONS to the above procedure regarding physical exams

New players to the club also need to submit a Uniform Order form:

• Uniform Order form - All NEW players must purchase a uniform jersey and shorts
  • Uniform jerseys will be $40.  XXL and larger will be $45.
  • Uniform shorts will be $35.  XXL and larger will be $40.
  • Replacement reversible practice pennies will be $21.
  • Returning players who want to replace a practice penny, uniform jersey, and/or uniform shorts, or are requesting a different uniform number, need to submit a Uniform Order form with proper payment.
  • Players will be assigned their jersey numbers starting at the December meeting after they have paid for their uniform.

*** - Uniform numbers available ONLY to inexperienced Freshmen players (as these numbers also belong to senior Varsity players)

Due date for Registration Fee and forms will be in mid to late January, 2009.  All forms and fees will be due at that time.  Forms and fees can be dropped off or mailed to the registrar:
