Registration fee for 2012 for returning players is $550.00. The fee for new players to the team is $575.00 and includes a practice pinnie to be used throughout the player's lacrosse career at NCHS. Players will also have to submit a $100.00 uniiform deposit fee (which will not be cashed) to ensure uniforms are returned at the end of the season. Your check will be returned at the end of the season as well.
Families with two or more players will receive a $100 discount on the second player (and each subsequent player after that). Discounts earned for Ribfest Logistics (July, 2011), Last Fling (September, 2011), and any other fundraising will help reduce a player's registration fee.
All players must complete and submit ALL of the forms below before being allowed to tryout and practice with the team:
- Player registration (all 3 pages), Waiver, Code of Conduct, Parent Duties, and Uniform forms
- Replacement reversible practice pinnies will be $25.
- Returning players who want to replace a practice pinnie need to submit a Uniform Order form with proper payment.
- New players will be assigned their jersey numbers in mid-March, 2012.
- Naperville Central Athletic Form (Your son can also obtain this form from the NCHS Athletics office. This is the preferred method as this form will be on heavier card stock preferred by the school. You can also use this PDF file.)
- Concussion Form - New for 2011 / 2012 school year. Player and parent must sign
- ISHA Physical form
- Players who played a spring, fall, or winter sport during 2011 can pick up a copy of their IHSA physical from (if needed) from the NCHS Athletics office (see Debbie Adams).
- All players MUST be covered by a physical exam that is less than one year old throughout the season. If the player's last physcial was:
- On or after June 1, 2011 - he is covered for the season and only needs to submit the IHSA physcial form to the team's registrar.
- Between March 1, 2011 and May 31, 2011 - he is able to start practices and games, but must get another physical and submit the IHSA physical form to the team's registrar before the previous physical is a year old.
- Before March 1, 2011 - player must have another physical exam and submit the IHSA physical form to the team's registrar before he is able to participate in practices.
NO EXCEPTIONS to the above procedure regarding physical exams
Due date for registration fee and forms will be February 6, 2012. All forms and fees are due at that time. Make checks payable to "Naperville Central Boys Lacrosse". Forms and fees can be dropped off or mailed to the registrar:

P.S. Refunds
It is the team's policy to provide a refund at the following percentages and times:
Prior to first practice - Full refund, less any expenses already incurred by the team for the player. These expenses could include practice penny, insurance, etc.
Player is cut from team during tryouts - Full refund, less any expenses already incurred by the team for the player (indoor time and insurance). The team will attempt to limit these expenses, especially if player cuts are expected due to the number of players trying out.
Prior to first game - 50% refund.
After first game - No refund (unless injured). No refund will be granted after the first game if the player
- Quits the team
- Is suspended by the team
- Or becomes ineligible to play due to grades (academic ineligibility).
